To support practice operations such as quality assurance, documentation, scheduling, and administrative functions.
Legal and Safety Requirements
When required by law, court order, or to prevent a serious threat to health or safety.
Use of Technology and Telehealth
Services are provided through secure, HIPAA-compliant telehealth and electronic health record platforms, including JotPsych. Reasonable safeguards are used to protect the confidentiality of your PHI.
Your Rights
You have the right to:
Access and obtain a copy of your medical records
Request corrections to your health information
Request restrictions on certain uses or disclosures
Request confidential communications
Receive a list of certain disclosures
File a complaint if you believe your privacy rights have been violated
You will not be retaliated against for filing a complaint.
